Medical Forms, Letters & Evaluations

Documentation visits are available for patients who need clinically appropriate forms, letters, or evaluations. This may include:

  • Work notes and school notes
  • Return-to-work or return-to-school documentation
  • FMLA paperwork
  • Short-term disability forms
  • Medical or mental-health accommodation letters
  • Medication-related forms or letters
  • Treatment participation or recovery-support letters
  • Clinical summary letters
  • Referral and care-coordination documentation
  • Clinical diagnostic evaluations
  • Forensic diagnostic evaluations, summary letters, or formal written reports

All forms, letters, and evaluations require an appointment and are completed only when clinically appropriate and supported by the evaluation, medical record, treatment history, and available documentation. Completion does not guarantee approval by an employer, school, insurance carrier, court, agency, or other third party.

What should I choose if I need paperwork today?

Book a Medical Forms, Letters & Evaluations visit if your main reason for scheduling is FMLA, disability paperwork, an accommodation request, a work or school note, a treatment letter, a clinical evaluation, or forensic documentation.

Please upload or submit your form, deadline, relevant records, and the requesting organization’s instructions before your appointment whenever possible. This allows for a more focused review and helps prevent delays.

What happens during a same-day visit?

Your visit begins with a focused review of your symptoms, medical history, medications, allergies, concerns, and goals. Nadine will determine whether telehealth is appropriate for your needs and discuss next steps.

Depending on the reason for your visit, care may include:

  • Focused clinical assessment and treatment planning
  • Medication review, adjustment, or prescribing when clinically appropriate
  • Laboratory, imaging, or referral orders when clinically appropriate
  • Review of available records, laboratory results, or imaging
  • Work or school documentation after an appropriate evaluation
  • Follow-up planning, monitoring, or referral for in-person or specialty care

Some concerns cannot be safely evaluated or treated through telehealth. If an in-person examination, testing, procedure, urgent care, emergency department evaluation, or specialist assessment is needed, you will receive clear guidance on the next appropriate step.
