Documentation Should Be Accurate and Clinically Supported

Medical paperwork can feel overwhelming, especially when you are already managing a health concern, symptoms, treatment, work, school, or family responsibilities. This service provides time to review your request, discuss the medical or mental health concern involved, and determine what documentation can be clinically supported.

A documentation visit is not simply a signature appointment. Nadine Opong is a dual board-certified Family Nurse Practitioner and Psychiatric Mental Health Nurse Practitioner who will review the full picture before completing any form, note, letter, or evaluation.

What Forms and Documentation Can Be Addressed?

Depending on your individual situation, this service may include:

  • Work notes and return-to-work documentation
  • School notes and return-to-school documentation
  • Excuse notes after an appropriate evaluation
  • FMLA and medical leave paperwork
  • Short-term disability forms
  • Medical and mental health accommodation letters
  • Functional limitation documentation

Documentation is completed only when it accurately reflects the clinical evaluation and available medical information. Completion of a form or letter does not guarantee approval by an employer, school, insurance company, court, agency, or other third party.

What Happens During a Documentation Visit?

During your telehealth appointment, your concerns and request will be reviewed in a respectful, private setting. The visit may include discussion of:

  • Your symptoms, diagnosis, treatment history, and current needs
  • How your condition affects daily activities, work, school, or functioning
  • Relevant medications, records, laboratory results, or specialist information
  • The specific form, letter, deadline, and requesting organization
  • Whether the request can be supported through telehealth or needs additional evaluation

If your request requires an in-person exam, testing, specialist records, an established treatment relationship, or care outside the scope of telehealth, you will receive guidance on the next appropriate step.

Work, School, and Excuse Notes

Work, school, and excuse notes may be provided after an appropriate clinical evaluation. Notes are based on the information available at the time of your visit and cannot be changed, backdated, or written in a way that misrepresents the medical record.

If you need a note for an illness, injury, mental health concern, medication issue, or follow-up appointment, choose the visit type that best matches your primary health concern. Documentation may be addressed during that appointment when clinically appropriate.

FMLA and Disability Paperwork

For established patients only. FMLA, leave, and short-term disability paperwork may be addressed when there is sufficient clinical information to support the request. These forms often require a focused discussion of symptoms, diagnosis, treatment needs, functional limitations, and expected follow-up.

Please submit forms, deadlines, instructions, and any supporting records before your appointment whenever possible. Third-party approval decisions are made by the employer, insurer, or agency, not by Swift Solutions Medical Center.

Accommodation Letters

Medical or mental health accommodation letters may be considered when clinically supported. Examples may include school, workplace, medication, treatment, or functional accommodations.

Some accommodation requests may require additional records, ongoing treatment history, specialist input, testing, or an in-person assessment before a letter can be completed.

Clinical and Forensic Evaluations

Clinical diagnostic evaluations are structured appointments used to assess symptoms, diagnoses, functioning, treatment needs, and next steps.

Forensic diagnostic evaluation options may be available for patients who need a clinical opinion, summary letter, or formal written report for an administrative, legal, probation, custody, disability, or court-related matter. These evaluations are clinical services and do not include legal representation or guarantee a specific outcome.

How to Prepare for Your Appointment

To help your visit go smoothly, please have the following available before your appointment:

  • The form, letter request, or paperwork needing completion
  • The deadline and instructions from the requesting organization
  • Relevant medical records, test results, discharge paperwork, or specialist notes
  • Your employer, school, agency, attorney, court, or organization contact information, if applicable
  • A clear explanation of what you are requesting and why

Forms, letters, and evaluations require an appointment and are completed only when medically appropriate. Telehealth may not be the right setting for every documentation request.
