Primary care visits may be appropriate for stable ongoing health conditions and health concerns, including:

What happens during a primary care visit?

Your visit begins with a review of your current concerns, medical history, medications, allergies, symptoms, and health goals. Nadine will discuss what is working, what needs more attention, and what next steps may be appropriate.

Depending on your needs, your visit may include medication review, refill planning, laboratory or imaging review, new orders when clinically appropriate, lifestyle guidance, treatment recommendations, or referral coordination.

The goal is to help you understand your health, make informed decisions, and feel confident about your next steps.

Can you help manage chronic conditions?

Yes. Chronic conditions often require consistent follow-up, medication oversight, laboratory monitoring, and a plan that fits into everyday life. Nadine provides ongoing support for stable chronic medical conditions and works with patients to identify practical goals for improved health and function.

Follow-up care may include reviewing symptoms, tracking progress, discussing medication effectiveness or side effects, monitoring relevant labs, and adjusting the care plan when clinically appropriate.

Primary care is not about being perfect. It is about having a provider who helps you stay informed, supported, and proactive.

Do you provide medication management and refills?

Yes. Primary care visits can include medication review, refill planning, medication safety discussions, and monitoring for stable medical conditions.

Up to seven medications may be reviewed during a visit. Refills are generally provided monthly. Ninety-day prescriptions may be considered when clinically appropriate and when relevant laboratory testing from the past 12 months is available and reviewed.

Some medications, conditions, laboratory needs, or new symptoms may require a follow-up visit, updated testing, in-person evaluation, or specialist involvement before treatment can be continued or changed.

Can you order or review labs and imaging?

Yes. Nadine can review available laboratory results, imaging reports, and medical records as part of your visit. Laboratory testing, imaging, or other diagnostic studies may be ordered when clinically appropriate.

If you have outside records, prior test results, or medication information that may help with your care, you may submit those documents before your appointment for review.

Can you help with referrals and care coordination?

Yes. When clinically appropriate, Nadine can help coordinate care by placing referrals, reviewing recommendations from other clinicians, and helping you understand what type of follow-up may be needed.

Some health concerns require an in-person examination, diagnostic procedure, specialist evaluation, or emergency care. When that is the safest next step, you will be guided toward the appropriate level of care.

Can you complete FMLA forms, work notes, or disability paperwork?

When clinically appropriate, Nadine may complete work notes, school notes, FMLA forms, short-term disability paperwork, medical letters, medication-related forms, and other clinical documentation.

Documentation must be supported by your evaluation, diagnosis, treatment plan, and medical record. A scheduled visit does not guarantee approval of a requested form, leave, accommodation, disability determination, order, or letter.
