FMLA Primary Care
FMLA documentation services at Swift Solutions Medical Center are available for established patients of the practice only.

When a medical condition is affecting your ability to work, keep up with daily responsibilities, or safely complete your job duties, it may be time to schedule a Primary Care or Chronic Care & Medication Management visit with Swift Solutions Medical Center.

FMLA and medical leave paperwork can feel stressful, especially when you are already trying to manage symptoms, appointments, medications, family responsibilities, and work expectations. The first step is not paperwork. The first step is an appropriate medical evaluation.

When should I book an FMLA appointment for a medical concern?

Book a visit when you have a health condition that is affecting your function at work and you need medical evaluation, ongoing treatment, follow-up, or documentation.

A primary care FMLA visit may be appropriate for concerns such as:

  • Chronic migraines or frequent headaches
  • Hypertension, diabetes, thyroid disorders, anemia, or other chronic conditions requiring ongoing care
  • Asthma or COPD symptoms affecting daily activity
  • Chronic pain, fatigue, dizziness, or sleep problems
  • PCOS, menstrual concerns, menopause symptoms, or other hormone-related concerns
  • Recovery from an illness, injury, surgery, hospitalization, or medical procedure
  • Medication side effects or medication changes that affect function
  • A flare of an established chronic condition
  • A need for medical leave, intermittent leave, workplace restrictions, or accommodation documentation

If you have a new medical issue, book the appointment as soon as possible. It is much harder to complete accurate paperwork after the fact when there has been no evaluation, treatment discussion, or documentation of the concern.

What happens during a primary care FMLA visit?

Nadine will review your symptoms, medical history, diagnosis, medications, treatment plan, records, job demands, and how your health condition is affecting your ability to function.

Depending on your needs, the visit may include:

  • A focused medical evaluation
  • Review of chronic conditions, medications, and recent changes in symptoms
  • Laboratory, imaging, referral, or follow-up planning when clinically appropriate
  • Discussion of work limitations, leave needs, or return-to-work planning
  • Review of FMLA, short-term disability, or accommodation forms when clinically supported
  • Clear guidance if an in-person examination, specialist evaluation, testing, or other level of care is needed

Not every condition can be evaluated or managed through telehealth. If your concern requires an in-person exam, procedure, urgent care, emergency care, or specialist support, you will receive guidance on the next appropriate step.

What paperwork can Swift help with?

When medically appropriate, Swift Solutions Medical Center may address:

  • FMLA and medical leave forms
  • Short-term disability paperwork
  • Work notes and return-to-work documentation
  • Workplace accommodation letters
  • Functional limitation documentation
  • Medical summary letters
  • Referral and care-coordination documentation

Forms are completed based on the clinical evaluation, available records, and treatment history. Completion does not guarantee approval by your employer, insurance company, leave administrator, or another third party.

How can I prepare for my appointment?

Please submit your paperwork, deadline, employer instructions, job description if available, relevant records, and a clear explanation of what you are requesting before your visit whenever possible.

Documents may be submitted to Info@swiftsolutionsmed.com before your appointment. Our team will review submissions and respond within 24–48 hours. For sensitive medical information, including medical records, laboratory results, imaging, or medication photos, submit documents only to Nadine@swiftsolutionsmed.com. This secure, send-only email is for document submission only and does not receive replies.

Your health comes first

You do not have to minimize symptoms just because work is busy or because other people are depending on you. When your health is affecting your ability to function, getting evaluated early can help you understand your options and make a clear plan.

Swift Solutions Medical Center provides telehealth primary care for patients of all ages when clinically appropriate. Book online 24/7 from home.

Important: Telehealth is not appropriate for emergencies. Call 911 or go to the nearest emergency department for chest pain, severe shortness of breath, stroke symptoms, severe allergic reaction, severe injury, or another urgent medical emergency.
