Book a visit when a mental health condition is affecting your ability to safely and consistently perform your job duties, and you need an evaluation, treatment plan, follow-up, or medically supported documentation.
A visit may be appropriate if you are experiencing:
You do not have to wait until you are completely burned out or in crisis to ask for help. The goal is to understand what is happening, create a treatment plan, and discuss whether medical leave or workplace accommodations may be clinically supported.
Nadine Opong, a dual board-certified Family Nurse Practitioner and Psychiatric Mental Health Nurse Practitioner, will review your symptoms, mental health history, current treatment, medications, work concerns, and how your condition is affecting daily functioning.
Your appointment may include:
FMLA paperwork is not simply signed because it is requested. Documentation must accurately reflect your evaluation, symptoms, treatment needs, functional limitations, and available records. Approval decisions are made by your employer or leave administrator.
Please submit your FMLA forms, employer instructions, due date, job description if available, and relevant treatment records before your appointment whenever possible. This gives the provider time to understand exactly what information is being requested.
For sensitive medical records, laboratory results, imaging, or medication photos, send documents to Nadine@swiftsolutionsmed.com. This secure, send-only email is for document submission only and does not receive replies.
Taking leave does not mean you are weak, lazy, or failing. Sometimes it means you are finally giving yourself room to stabilize, rest, attend treatment, and come back with more support in place.
